We have been working alongside the six Health and Social Care Partnerships to host events to raise awareness about the Moving Forward Together Programme and illustrate where this aligns to local plans and priories for the delivery of services. More importantly we have been asking people what matters most to them so that when we develop future models of health and social care they are person centered and meet people's needs.
